 Another firm has been added to the lists of those who now realise that accelerating development may well cause employees to withhold thoughts that could postpone the decreed launch date. Samsung's Note 7 won't be the last launch that suffers this way.

The solution is easy. Once you see that this is a governance issue, then you can design a project governing system that takes account of human fears and emotions by creating processes for their expression. And then projects need not explode in your face.
